Sykehuset i Vestfold HF søker en engasjert eldrehelsekoordinator i 100% stilling knyttet til kommunene i Vestfold. Du vil samarbeide tett med geriater og tverrfaglige team for å sikre oppfølging etter utskrivelse og utvikle det nye tilbudet til eldre som lever med skrøpelighet.

Du har norsk autorisasjon som sykepleier, GVK eller tilsvarende videreutdanning og erfaring fra tverrfaglig arbeid. Vi tilbyr fagmiljø, kurs og god oppfølging i stillingen.
